2022 Covid-19 Supplementary Forms

Philmont recommends that anyone who has been hospitalized for Covid-19 since the beginning of the pandemic be cleared by their primary care provider. Philmont recommends that participants who had a positive Covid-19 test or Covid-19 illness, regardless of severity, within 6 months of scheduled arrival at Philmont be cleared for high levels of exertion at high elevations by their primary care provider.

Philmont Backcountry Participants & Staff

Parts A, B, and C are required for each participant involved in a trek, cavalcade or any backcountry experience that involves a Tooth of Time hike or spending the night in the backcountry. This includes Mountain Trek, Philmont Sneak Peak, NAYLE, PLC, and NAYLE and PLC Staff. This is also the form required for all seasonal staff.

The Immunization Exemption Request form is optional.

Philmont Training Center Participants

Parts A and B are required for all participants attending BSA Family Adventure Camp, PTC Conferences, and the following youth conference groups: Small Fry, Bandits, Mustangs, and Trailblazers. These groups have limited hiking and backcountry time. Participants in Mountain Trek, Philmont Sneak Peak, NAYLE, PLC, and NAYLE and PLC Staff are required to complete Part C as well.
